Instructions for Single Signature
Signers and witnesses must be registered New York City voters.
If you're not, you can download a registration form online and mail it
in to the Board of Elections (see link on the left). Then follow these
instructions:

Right-click here
and choose "Save As" to download the petition form to your
computer.

Print 2 copies of the petition form. One is for you and the other
is for the person witnessing your signature.

[Note: Your name and address should be exactly as it is on your
voter registration form.] Using blue ink, on page 1 of the first
copy sign your name on the top line of the left side, then neatly
print your name right below that. Then neatly print your street address
with apartment number on the top line to the right, and your borough
and zip code below. Lastly print your county on the far right and
date on the far left. Then sign on page 2 and 3 in the signature line
at the bottom of the page.

A witness is required, so have another registered voter witness your
petition by filling in their signature under Statement of Witness.*

Have your witness fill out a petition following step 3 above, then
you sign as their witness.

Mail in both petitions to NYC 9/11 Ballot Initiative, 1173A
Second Avenue, Suite 155, New York, NY 10065.

Please print more copies of the petition and commit to having another
5 people do steps 1-5 and repeat the process to their friends and
families.

Please act immediately to ensure our chances for
success. If you act now, we can get this initiative into the Nov.
6th 2008 general election.

You can help this urgent cause by gathering multiple petition
signatures. First, you may want to have an overview to show people. We

This describes a few easy ways to get people to sign the petition.

"Save As" to download it to your computer. After getting 3
signatures, fill in your signature under Statement of Witness, then
mail the form to the address on page 3 of the petition.
Note: Many people are collecting petition signatures from
people they know in their neighborhood or workplace. This can easily be
done alone. We also need volunteers to go to key locations throughout
the 5 boroughs. This is best done in small teams that can set goals and
